About this grant

Supports community and faith-based organizations in Washington State that promote brain health, increase awareness of dementia and Alzheimer's disease, and connect people with resources. Organizations are expected to train trusted messengers, host educational sessions using a provided curriculum, and share information about dementia risk factors with their communities. Priority communities include Black and African American, American Indian and Alaska Native, Hispanic, and rural communities.

What you get

Minimum grant of $5,000 for completing required activities and deliverables
Optional additional $2,500 for completing one optional activity (maximum $7,500 total)
Support and resources from DOH throughout the grant term
Facilitator training for the Brain Health and Dementia Awareness in our Communities (BDAC) curriculum
Reimbursement for completed contract deliverables upon invoice submission

How to apply

Identification of 1-2 trusted messengers from the community
Trusted messengers must attend a full-day in-person facilitator training
Host at least 2 educational sessions using the BDAC curriculum
Submit online survey results and scanned sign-in sheets for each training
Fill out a DOH online activity report
Submit a final narrative report summarizing grant activities and outcomes
Complete project evaluation survey sent by DOH

Eligibility

Must be a community or faith-based organization in Washington State
Must aim to promote brain health and increase awareness of dementia
Must connect people with information and resources about dementia
Must encourage partnerships and collaboration to address brain health and dementia
Must abide by all federal grant requirements (CDC BOLD Infrastructure for Alzheimer's Act)
