About this grant

Supports projects addressing health and wellness issues in specific counties of Ohio and West Virginia, with priority areas including healthy eating and active living, mental health and addiction, and thriving neighborhoods. Eligible applicants include 501(c)(3) organizations, churches, government agencies, and fiscally sponsored groups undertaking charitable projects. Funding can support project/program activities, equipment, capacity building, capital improvements, or general operating needs.

Tax return type (990 N, 990 EZ, 990, or not required)
Fiscal sponsor letter (if applicable and first-time applicant)

Eligibility

Must be a 501(c)(3) organization, church, government agency, or use a fiscal sponsor
Project must serve at least one county in the SHF geographic service area (Athens, Meigs, or Washington County OH; or Calhoun, Jackson, Pleasants, Ritchie, Roane, Tyler, Wirt, or Wood County WV)
Project must address a priority area: Healthy Eating/Active Living, Mental Health and Addiction, or Thriving Neighborhoods
Municipalities and county governments are not eligible for flexible funding (though may apply for standard grants)
Flexible funding requires prior SHF grant receipt and 501(c)(3) or public entity status
