About this grant

Funds the administration and oversight of a community grant program to support implementation of the Tennessee Maternal Health Strategic Plan. The selected grantee will design and deliver at least two initiatives focused on improving maternal healthcare access, strengthening maternal health infrastructure, expanding the maternal health workforce, and addressing social determinants of maternal health. All initiatives must be carried out in partnership with local organizations and prioritize underserved, rural, or high-risk populations.

Eligibility

Must administer and oversee a community grant program aligned with the Tennessee Maternal Health Strategic Plan
Must match $300,000 in philanthropic funds held in a designated maternal health depository
Must design and deliver at least two initiatives aligned with MHSP focus areas
All initiatives must be carried out in partnership with local organizations
Must focus on underserved, rural, or high-risk populations
Must maintain a financial tracking system documenting grant expenditure and matching funds
Must submit quarterly fiscal and programmatic reports
All activities and final deliverables must be completed no later than September 30, 2027
