About this grant

Supports rural Kansas healthcare organizations integrating Community Health Workers (CHWs) into clinical settings to deliver Accountable Food Is Medicine (AFIM) interventions targeting chronic disease. Year 2 features expanded evaluation support, required reporting, sustainability planning, and development of public/private payment integration for CHWs. Up to seven proposals will be funded under Kansas's Rural Health Transformation Plan.

Eligibility

Must be a rural, Kansas-based healthcare organization
Eligible types include FQHCs, rural health clinics, hospitals, local health departments, Native American Sovereign Tribe healthcare facilities, certified community behavioral health clinics, FQHC look-alike clinics, and primary care clinics
Must possess an active Unique Entity Identifier (UEI)
Must possess a Federal Employer Identification Number (FEIN)
Must possess a SMART Vendor ID
