About this grant

Supports nonprofit organizations delivering projects that address priority health needs identified in local Community Health Needs Assessments, with a focus on vulnerable and underserved populations. Funded projects must promote health equity, strengthen collaborative service systems, and operate within the service areas of participating CommonSpirit Health hospitals. Projects must run between 9 and 12 months and address areas such as access to care, chronic disease management, mental health, substance use, or social determinants of health.

How to apply

Complete online grant application submitted by deadline (no emailed applications accepted)
Project budget including all funding sources
IRS Determination Letter documenting 501(c)(3) status
Description of collaborating partner organizations (for grants of $20,000 or more)
Proposed project activities aligned with hospital's CHNA health priorities

Eligibility

Must be an IRS 501(c)(3) nonprofit organization or have a 501(c)(3) fiscal agent capable of administering grant funds
Must provide an IRS Determination Letter documenting 501(c)(3) status and be in good standing
Applicants and partners must have a philosophy and services not inconsistent with CommonSpirit Health's Mission, Vision and Values
Proposals must address one or more significant health needs from the hospital's most recent CHNA or Community Health Implementation Strategy
People and communities served must be within the hospital's service area and include underserved or vulnerable populations
Proposals for grants of $20,000 or more must include collaborating partner organizations with distinct, complementary, and substantive project roles
Projects must be for one-year periods only; projects will not be funded for more than four consecutive years
Project duration must be between 9 and 12 months
