About this grant

Provides restricted charitable contributions to nonprofit organizations for defined projects that address significant community health needs identified in hospital community health needs assessments. Projects must support service delivery and strengthen collaborative service systems for underserved or vulnerable populations within participating hospital service areas. Grants are awarded for one-year project periods.

Eligibility

Must be an IRS 501(c)(3) nonprofit organization, or have a fiscal agent that is a 501(c)(3) organization capable of administering grant project funds
Proposals must address one or more significant health needs in the hospital's most recent community health needs assessment (CHNA) or implementation strategy
Project activity must be sustained over a period of between 9 and 12 months
Proposals for grant projects of $20,000 or more must include collaborating partner organizations with distinct, complementary, and substantive project roles
People and communities to be served must be within the hospital's service area and include identified underserved or vulnerable populations
Proposals are for one-year periods only; projects will not be funded for more than four consecutive years
Ineligible proposals include unrestricted general operating support, event sponsorships, and activities less than 9 months in duration
Capital projects, lobbying, political campaigns, and contracts for services are not funded
