Grant for Health Equity Collaboratives

Supports collaboratives in Arizona implementing systems change projects that address root causes of health disparities by improving policies, practices, resource flows, and power dynamics. Grants fund three-year implementation efforts aimed at creating lasting, widespread improvements in health outcomes across communities experiencing inequities. Applicants should have completed a planning process or otherwise be ready to implement their proposed systems change.
