Grant for Community Nonprofits and School Districts

Provides small grants to support discrete programs, meetings, or training expenses that address a community need aligned with the social determinants of health. Funding is intended to build organizational capacity, particularly for nonprofits engaging with marginalized populations, and to increase collaboration among community-based organizations. School districts or their foundations may apply on behalf of individual school buildings.
