About this grant

A collaborative grant program supporting nonprofit and public entity programs that improve health and well-being across the South Coast region of Massachusetts and Rhode Island. Funded programs must align with the 2025 Community Health Needs Assessment priorities, which include housing, food access, behavioral health, chronic disease, maternal and child health, and socio-economic factors. Grants may support existing programs, program expansion, or operational costs directly tied to program delivery.

What you get

Grant award between $10,000 and $25,000 for a one-year grant period
Support for personnel costs (salary, benefits, payroll taxes)
Support for program supplies and materials
Support for other reasonable direct and indirect program costs

How to apply

Completed Letter of Interest (max 2 pages, submitted August 17–September 28, 2026)
Completed Request for Proposal (by invitation only, mid-October notification)
Participation in interview and/or site visit if required
Organizational budget, audit, and financial statements
Description of program outcomes and performance indicators

Eligibility

Must be a 501(c)(3) nonprofit organization or public entity (municipality, school, or public agency)
Programs must benefit residents within the Southcoast Health and/or United Way of Greater New Bedford service areas
Only one application per organization (limited exceptions may apply)
Applicants may apply for up to one year of funding
A clear backbone or lead fiscal agent must be identified and responsible for monitoring and reporting
Program must align with at least one priority in the SoCHA 2025 Community Health Needs Assessment
Organizations currently receiving Community Impact grant funding must have completed all required reporting to be eligible
