About this grant

Supports Wisconsin-based organizations and community-academic partnerships working to improve public health, advance health equity, and develop the health workforce across the state. Funding is available for community-led partnerships, nonprofit organizations, government entities, and Medical College of Wisconsin-affiliated projects. Programs range from small learning event support grants to larger seed and momentum grants for health improvement initiatives.

What you get

Up to $5,000 for Learning Event Support (community-led grants)
Up to $50,000 for Seed Grants (12-month projects)
Up to $250,000 for Momentum Grants (24-month projects)
Access to pre-application consultation with AHW team
Community-academic partnership pairing with MCW faculty (for eligible community applicants)
Technical assistance resources

Online application form (no paper or emailed applications accepted)
Project budget
Description of key learning gap or health need addressed
Identification of targeted participant group and health equity impact
DocuSign signatures from key individuals listed in the application
Listing of current funding sources (for supplanting compliance)

Eligibility

Must be a Wisconsin-based 501(c)(3) or 501(c)(6) organization, or a Wisconsin government-based entity
Organization must have an EIN number
Host organization is responsible for fiduciary and reporting requirements
AHW does not accept unsolicited proposals — must apply to an open RFA
Applications must be submitted through AHW's online grants management system
Funds may not be used to supplant funds or resources available from other sources
