About this grant

Supports collaborative initiatives between two or more organizations working to improve maternal health outcomes and advance health equity for pregnant and postpartum individuals and their families. Projects should address systemic barriers, strengthen community supports, and create sustainable solutions in the Greater Waterbury region. Selected grantees also participate in a facilitated learning and implementation cohort.

Share of up to $150,000 in grant funding for one-year collaborative projects
Participation in the Advancing Maternal Health Collaborative facilitated learning cohort
Four facilitated collaborative sessions (three in 2026, one in 2027)
Shared learning with peer organizations
Ongoing implementation coaching tailored to cohort needs
Human-centered design activities with community members

Description of the partnership and why it is necessary
Role of each partner organization
Description of how decisions will be shared
Project budget reflecting true cost of proposed work
Demonstration of alignment with maternal health priorities
Description of community need and equity-centered approach
Clear outcomes and measurable impact
Plan for sustainable partnerships beyond the grant period

Eligibility

Must be a nonprofit organization, public agency, or community group serving residents of Greater Waterbury
Applications must include at least two organizations working together toward shared goals
All organizations must participate in the Advancing Maternal Health Collaborative facilitated by KJR Consulting
Projects must align with one or more maternal health priority areas
Organizations serving populations disproportionately affected by maternal health inequities are strongly encouraged to apply
