About this grant

Funds organizations working to address and improve health outcomes for populations experiencing health disparities across eight priority health areas in Minnesota. Projects must demonstrate a strong rationale, alignment between approach and the health disparity being addressed, and a strategy for upstream impact. Community Health Boards, Tribal Nations, and nonprofits serving underserved communities are among the eligible applicants.

Eligibility

Must address health disparities in one of eight priority health areas
Community Health Boards and Tribal Nations do not need to submit the Due Diligence Review Form
Must submit all required application forms including Application Form, Work and Evaluation Plan, Budget Template, Due Diligence Review Form (if applicable), and Conflict of Interest Disclosure Form
