Supports Shalom Zone ministries, emerging ministries, connectional agencies, and health ministries including parish nurse programs within the Iowa Annual Conference of The United Methodist Church. Disclosure of Matthew 25 Funds requests and District Askings
Disclosure of additional grant requests for the year
Payment and mailing information
Ministry budget (common document, spreadsheet, image, presentation, or PDF formats; max 25 MB)
List of other funding sources and amounts received
Authorized signature

Eligibility

Must be a Shalom Zone ministry, emerging ministry, connectional agency, or health ministry (including parish nurse programs) within the Iowa Annual Conference
Must identify a local church or organization and an Iowa Annual Conference district
Must describe ministry goals for 2027 and alignment with Iowa United Methodist strategic priorities
Must disclose other grant requests and funding sources
Must attach or insert a copy of the ministry's budget
Application must be signed by an authorized person from the church or organization
Recipients must submit year-end reports; failure to do so results in ineligibility for future funding
