About this grant

Funds multi-family affordable housing projects that incorporate elements connecting communities to health and social services, supporting Medicaid-eligible individuals and families earning up to 80% AMI across Arizona. Projects must be shovel-ready, demonstrate cross-sector partnerships, and address social determinants of health. Both nonprofit and for-profit affordable housing developers are eligible.

Application narrative (no more than five pages) covering comprehensive community development, social determinants of health, and serving AHCCCS members
Comprehensive Community Development Plan (if applicable)
MOUs or similar agreements with SDOH collaborators (if applicable)
Construction budget
Brief construction timeline
Sources & uses statement
Proof of all committed financing
Project proforma (if applicable)
Financial statements for the most recent three years (audited for nonprofits; federal tax returns for for-profits)

Eligibility

Must be a multi-family affordable housing project (home ownership projects not eligible)
Project must serve families or individuals earning up to 80% AMI
Project must be 'shovel ready' — fully or substantially completed projects are not eligible
A substantial portion of financing must be committed and demonstrated in the application
Project site must be owned, under purchase agreement, or long-term lease
Project must demonstrate deliberate connection to community assets and cross-sector partnerships
Number of units built or renovated must be significant to the local community
Project must be located in Arizona
